This form is to apply for free legal aid. The information you give us is confidential under the Minnesota Rules of Professional Conduct for lawyers. The information you give us in this form is sent electronically to the legal aid office that handles cases in your county. We keep a copy of your information for a short time as a back-up and to report to our funders how many people use the system. When you fill out this form, you are saying it is okay for us to keep the information for a short time and to send your information to the legal aid office that handles cases in your county.

Who else gets your information:

The office that gets your application for legal help uses that information to see if they can help you. They also use it to work on your case if they decide to help. The office that gets your application may keep a record of it as part of their data practices. If they do, your information is kept privately and is only shared if a court or judge requires it.
